On my Xbox 360 I sometimes get messages saying, your disc is unreadable?

What causes this...is it that my disc is scratched (which it isn't scratched much at all)...or is it that my Xbox 360 needs to be fixed...this error message probably comes up about 3 times a week (not in one day but every now and then)
